Insurance Benefits:

I am currently accepting clients as an In-Network Provider with Premera, Regence, Blue Cross Blue Shield, Lifewise, Aetna, Kaiser Permanente, as well as an Out-of-Network Provider with other insurance companies. 
Helpful information to know about your insurance benefits:
* Am I covered for mental health counseling with Sharon Sanborn (Is this In-Network or not? Primary or Secondary)?
* If applicable, am I covered for couples/family counseling (CPT code 90846 or 98047)?
* Which insurance company should claims be submitted to?
* At which mailing address is the claim to be submitted to?
* Do I have a deductible? How much? If yes, have I already met it or not?
* How much co-pay or co-insurance do I owe per visit?
* How many sessions am I covered for per year? Start date? How many sessions have I used since start date?

Costs and payments:

I accept Credit, Debit and HSA Cards (with transaction fee), Cash or Checks or E-Checks (no transaction fee). All payments are due at the time of service, either in person or over the Phone/Zoom/Skype.
